Prior to starting the application, be prepared and please ensure the following items are available.

ALL Front and back copy of Driver’s license
ALL Copy of medical certificate
ALL Copy of social security card

ALL Last 10 years employment history with explanation for any gaps in employment
ALL Names of motor carriers that you were employed by or drove under the authority of including contact names and numbers

O/O current 2290 if requesting apportion plates
O/O current annual DOT inspection (within most recent 60 days) for truck and trailer
O/O current vehicle registration for truck and trailer

O/O If driver has Class C license, provide scale ticket for unladen actual weight of vehicle (with full fuel tanks)
O/O pictures of manufacturer’s plate/sticker with GVWR for truck and trailer
